Position Summary:
\nThe Transition Nurse facilitates successful transition from a person’s present living environment (i.e. Hospital in

5 or more years of experience working with clients with various types of dementia, or other neurological conditions who are experiencing a responsive behaviour and/or family members/caregivers of these seniors

Language: Proficiency in both official languages an asset

Hours of Work:
\nThe work week is 40 hours and will usually be worked Monday to Friday from 9 am to 5 pm. Occasionally, based on client need, it may be necessary to flex these hours. Additional flexibility may be required to allow for staff training, client/agency emergencies, special agency functions and other responsibilities.\n
