Placement within the grid will be determined in accordance with company policies and procedures, as well as the candidate’s knowledge, skills, qualifications, and relevant experience.

Professional assessment of patients in the community and in their homes
\n
Develops and maintains an effective Care Plan with respect to Community Health Services, independently or in cooperation with the Ontario Health atHome

Works with the patient on appropriate time
Position Details

Language: specific, measurable goals and outcomes in collaboration with the care team

Works independently and as an integral member of an interdisciplinary care team
\n
Provides active guidance for workplace Health and Safety
\n
Master’s Degree (or equivalent) degree in Physiotherapy from a recognized university
\n
Registration with the College of Physiotherapists of Ontario
\n
Proven computer skills, including email, word processing, work scheduling systems
\n
Promotes independence of the client in the home and in the community A reliable vehicle for some regular travel
\n
Excellent English verbal and written communication skills
\n
French language skills are an asset
\n
