Employment Type: The pharmacist is responsible for providing pharmaceutical care and services to CHSLD Wales Inc. residents. They collaborate with various teams to provide quality care and services that respect a milieu de vie approach and residents' rights. They maintain a partnership with the community pharmacy and contribute to the continuous quality improvement of pharmaceutical care and services.

**This is a temporary position until March 2027, with the possibility of renewal and permanent employment.

Responsabilities

Carry out the comparative medication review (BCM) in collaboration with the healthcare team
\n
Meet with residents and their loved ones to discuss pharmacotherapy
\n
Evaluate pharmacotherapy
\n
Develop the pharmaceutical care plan and agree on priorities in collaboration with the care team, the resident, and their loved ones
\n
Implement the pharmaceutical care plan and adjust as needed
\n
Provide quality comfort care in collaboration with the care team
\n
