based nursing experience with 2

disciplinary team that includes family physicians, nurse practitioners, clinical nurses, chiropodists, registered dietitians, counsellors, physiotherapy and community health team staff
4. Skills/experience to fulfil major functions of role: wellness care; assessment, diagnosis and management of episodic illness; managing chronic disease; and group work.
5. Strong communication, organizational, and issue management skills.
6. Excellent interpersonal and problem

call on shared schedule with 15 other health centre physicians and nurse practitioners.
8. Knowledge of/experience with health issues across the lifecycle. Specific expertise in the following areas would be considered an asset: geriatrics, palliative care, mental health, chronic disease management, maternal and infant/child health as well as familiarity with working in a rural setting.
9. Strong facility with computer; previous experience with electronic medical records would be an asset.
10. Car required for home visits.
