Required Experience

day leadership and operational oversight for the Mobile Wellness Team, including intake, care coordination, and clinical service delivery Participate in policy development, program planning, and continuous quality improvement initiatives Support team members (e.G., navigator, social workers, nurses) through case consultation, mentorship, and clinical guidance Coordinate service delivery schedules, ensuring efficient use of resources and timely responses to client needs Ensure new referrals are addressed promptly and appropriately Monitor and evaluate program outcomes and quality indicators to ensure alignment with organizational and funder expectations Provide consultation and support for complex cases, including crisis situations and mental health or addictions concerns Support the integration of traditional and cultural healing pathways in collaboration with Elders, Knowledge Keepers, and Traditional Healers Uphold principles of trauma‑informed, holistic, and culturally safe care Ensure services are delivered in accordance with ethical, legal, and professional standards of practice Strengthen partnerships with local health and social service agencies, hospitals, and First Nation communities Participate in community outreach, education, and promotion of the Mobile Wellness Program Represent Noojmowin Teg Health Centre at inter‑agency meetings, community committees, and collaborative initiatives Support coordination between Mobile Wellness services and other internal programs to ensure seamless care pathways Maintain accurate documentation and data entry within the Electronic Medical Record (EMR) Compile reports, statistics, and outcome data as required by funders and the organization Ensure adherence to organizational policies, procedures, and professional standards Support onboarding, supervision, and professional development for new and existing team members Perform other duties as assigned. Qualifications Mandatory Current registration and good standing with a recognized regulatory college (e.G., Ontario College of Social Workers and Social Service Workers, College of Nurses of Ontario, College of Registered Psychotherapists of Ontario, or other relevant body) Bachelor of Social Work degree, or another health‑related degree with relevant experience Minimum of 3 years of clinical experience in mental health and/or addictions services Demonstrated leadership, supervisory, and coordination experience Valid Class G driver’s licence, access to a reliable vehicle, and clear driver’s abstract Clear Vulnerable Sector Criminal Record Check Preferred Experience working with First Nation communities Familiarity with electronic medical record systems Knowledge of community resources and service providers within the Manitoulin Island region Ability to speak Anishinaabemowin considered an asset Knowledge, Skills, and Abilities Robust leadership and team facilitation skills Knowledge of serious mental illness, crisis intervention, and trauma‑informed care Understanding of Anishinaabe culture, traditional wellness, and Indigenous models of health Excellent interpersonal, organizational, and communication skills Ability to work collaboratively across disciplines and with minimal supervision Work Conditions Services delivered across seven First Nation communities and regional sites Frequent travel and occasional evening or weekend work may be required Combination of office‑based, community‑based, and mobile service delivery environments #J
