Additional Required Qualifications:Minimum of two years' experience within the last five years of Home Care experience, although skills and experience relevant to nursing practice in community sector will be considered. An excellent attendance record, reliability and personal suitability must be demonstrated through your employment references. Physically able to perform all assigned tasks and may involve physical exertion not limited to climbing stairs, walking outside, lifting, pulling, pushing, stooping, and carrying which may involve visits to client homes that are not ideal. Preferred Qualifications:Experience in providing case management services with knowledge of Connect Care PCS and service authorizations. Experience in Home Parenteral Therapy (HPT), wounds, lower limb assessments, skin and ostomy care, palliative support and client teaching.
