RN License with the CNO in Good Standing required or RSW required
OTHER KNOWLEDGE, SKILLS AND EXPERIENCE
· Minimum 5 years demonstrated expertise in working with older adults with mental health illness.
· Certificates with PIECES, DementiAbility, and GPA are an asset.
· Extensive knowledge, skills and experience in the assessment/care planning for older adults and the use of clinical assessment tools as they relate to complex physical health problems. This may include pain, chronic disease management, delirium, metabolic disorders, cognitive function and impairment including dementia as well as other neurological disorders.
· Experience working in a LTCH or mobile role is an asset.
· Experience in leadership, mentoring or educator roles an asset.
· Superior communication skills (verbal, written and interpersonal) and use of personal

· Demonstrates ability to prioritize workload to meet established program requirements. Demonstrates ability to work independently, as well as function within an interdisciplinary team.
· Proficiency in the use of Microsoft 365 and use of electronic medical records.
· Valid G Driver’s licence, insurance and the ability to travel across the HNHB regions/ assigned areas.
Pay: From $43.93 per hour
