What you’ll do
Provide comprehensive wound assessments and treatment plans
Deliver advanced wound care services, including dressing selection, treatment protocols, and ongoing monitoring
Perform debridement and other procedures within your scope and credentialing (training and support available)
Coordinate care with facility nursing teams and leadership
Document visits clearly and efficiently in our system
Communicate recommendations and follow‑up plans with the care team
Work setting
Partnered facilities in the Grand Rapids area (SNF, rehab, assisted living, and similar settings).
Structured rounding schedule based on patient needs and facility census.
Supported onboarding, clinical guidance, and operational coordination.
Qualifications
Active Nurse Practitioner/Physician Assistant (NP/PA) license.
Solid independence, communication, and organizational skills.
A passion for patient‑centered care.
Clinical experience in wound care, SNF/rehab, home health, or related settings is a plus.
Strong clinical judgment and communication skills.
Reliable transportation for local travel between facilities.
Preferred (not required)
Wound Care Certification (preferred, with CEUs provided if needed within 90 days of hire).
Experience with wound documentation and treatment planning
Compensation and Advantages
Competitive Pay: $140,000 annually with bonus opportunities
