Hours: disciplinary specialized team whose purpose is to ensure that persons with severe and persistent mental health problems and/or substance use disorders have access to timely, responsive, evidence based psychosocial rehabilitation across the continuum of available services. Performs a variety of life skills, advocacy, supportive counseling, psychosocial rehabilitation planning, and general support duties involving clients who are living in their own housing for the assigned client population(s).
Reporting to the HCIS Manager (or designate), working within a psychosocial rehabilitation/harm reduction framework and informed by existing psychiatric and other health professional assessments, the Community Mental Health Worker 2 provides basic case management, establishes rehabilitation objectives and/or psychosocial rehabilitation plans for the assigned client population(s) and is expected to engage clients in various community settings as people with full potential and as meaningful collaborators in their own service planning, delivery and evaluation.
Scheduling Information:
Shift types are days and shift times are from 08:30 to 16:30 hours. This is a Casual opportunity. Casual means there are no guaranteed hours and work requirements can vary from 0
