Are you an experienced registered practical nurse seeking a rewarding career that cares for others, in a professional practice that cares for you? You’re looking in the right place.
As a Community Independence Care Coordinator (Status: Full-Time; Effective: Immediately; Location: Moosonee), you will be responsible for assessment and service planning for low needs patients with a focus on patient independence through system navigation and referral to appropriate community resources. The CICC will work in consultation with the Care Coordinator on complex patient files and where services are outside the scope of the RPN. This role will also perform direct nursing tasks, as assigned.
Whether you work in our office, in the community, or a health care facility – you will play a lead role in providing connected, accessible, patient-centred care – and be supported by our collaborative team that includes over 9,000 regulated health care and other professionals.

What will you do?
- In collaboration with patients and their families, assess care needs, determine eligibility for services, and develop individual care and service plans
- Link patients with service providers
- Coordinate and monitor care plan delivery
- Establish a helping relationship with patients and their families
- Balance patient needs and choices with available resources, ensuring patients’ values and preferences are respected
What must you have?
- Registered Practical Nurse in good standing with the College of Nurses of Ontario;
- Relevant experience in health care and Community Support Services;
- Understanding of issues and priorities within the health care sector;
- Knowledge of tools, systems, and databases used in health care service delivery and management;
- Knowledge of community based programs;
- Practical knowledge and understanding of relevant legislation;
- Effective planning, organizing, and evaluation skills to manage multiple patients and provide information reports upon request;
- Ability to accurately complete required documentation, reports, forms and recommendations;
- Working knowledge of computer software and Microsoft Office applications;
- Strong interpersonal skills to work with diverse patient groups with varying levels of comprehension and language capability;
- Effective communication skills to establish and maintain wide range of contacts with professional and organizations within the community;
- Solid collaboration skills to participate on projects and committees with colleagues across the area and with local community agencies and health professionals;
- Effective listening and strong facilitation skills to work with patients and their families/caregivers/SDMs in order to ensure best possible service is provided to the patient;
- Ability to communicate using various software programs to clearly articulate patient status;
- Ability to effectively deliver information relation to community services;
- Advanced oral and written proficiency in English is essential.

- Experience working with diverse patient groups, e.g., multicultural, homeless, palliative, acquired brain injury (ABI), mental health, geriatrics, pediatrics
- Case management experience or recent related community experience
