Outreach Employment & Life Skills Case Manager
Piers is seeking an Outreach Employment & Life Skills Case Manager to support individuals facing complex barriers to employment, including those experiencing or at risk of homelessness, mental health challenges, and substance use.
This role is community-based and outreach-focused, working directly with participants in various settings to support movement toward employment, peer mentorship training, and community inclusion.
Key Responsibilities
- Provide one-on-one and small group support to individuals to build life skills, employment readiness, and independence
- Conduct outreach in the community to engage individuals who may not access traditional office-based services
- Complete intake, assessments, and collaboratively develop individualized action plans with participants
- Support participants in addressing barriers to employment, including housing, basic needs, and access to community services
- Facilitate or coordinate life skills workshops and employment-related training
- Identify and coordinate short-term training and certification opportunities aligned with participant goals
- Build and maintain strong relationships with community partners, employers, and service providers to support participant outcomes
- Support participants in securing and maintaining employment, volunteer, or community-based placements
- Provide on-site or ongoing support to participants in placements as needed, including coaching and problem-solving
- Maintain accurate and timely case notes, documentation, and reporting in accordance with organizational and funder requirements
- Contribute to program evaluation, reporting, and continuous improvement efforts
- Ensure all work aligns with trauma-informed, culturally safe, and person-centered practices
- Maintain confidentiality and adhere to FOIPPA and organizational policies
Additional Responsibilities
- Support office operations as required, including opening/closing procedures and general administration
- Participate in team meetings, case reviews, and ongoing training
- Collaborate across programs to support participants and organizational priorities
- Other duties as required
Requirements:
- Must have a valid driver’s license, reliable vehicle, and appropriate insurance. This role requires regular travel within the community to support participants in various settings.
- Post-secondary education in Social Work, Human Services, Community Support, or a related field is preferred.
- An equivalent combination of education and relevant experience will be considered. Candidates with solid frontline experience working with individuals facing complex barriers will be prioritized.
- This position is dependent on a Criminal Records Check (Vulnerable Sector)
- Proficient in Microsoft 365, including Word, Excel, Google Drive, and other relevant platforms;
Job Types: Full-time, Fixed term contract
Contract length: 24 months
Pay: $25.00-$27.00 per hour
